Pre-Appointment Info Package

Thank you, you have successfully sent in your retainer payment to book your appointment with Acoustic Ink Tattoos!

If I haven't already, I will follow up with a confirmation booking via a calendar invite within a few days.

Before your appointment please go through this pre-appointment info package and complete the required tasks. You will find information on how to best prepare for your appointment, what to bring, as well as my waiver for you to fill out.


There are no refunds on retainers, but I'm always happy to hold onto or transfer funds in line with my policies.

Getting here:
The studio is located at 710 Fourth St. E. When you arrive, you will be going to the backyard along the left (carport) side of the house. Go through the wooden gate and up the steps to the REAR PORCH DOORS.
The studio is SHOES OFF, there is a mat inside the door for your shoes.

Bringing Items:
My studio is small, so I ask that you please limit the items you bring with you. You are welcome to bring such things as a book, headphones, a devise, your phone, a screw top beverage, and a packaged snack. 
Please do not bring open top beverages, take out food, or anything that can’t be sealed and stored.
 

Hygiene & Attire:
Please take the time to shower, and please shave an area larger than you think where the tattoo is to go. Wear fresh clothes that are comfortable and loose fitting with no sharp objects. White is not the best option, wear something that you don’t mind getting stained, this does happen.
 

Bringing Friends:
My studio has space for you to have up to 2 friends or family with you. It is crucial that all guests are respectful, do not disturb me while I’m working, and be 16 years or older. No children are permitted. No one will be permitted if sick.
If I have a guest artist in the studio, there is not enough room for company, sorry.
 

Health:
Please DO NOT come in if you are sick. If I get sick, I can’t work. If you arrive and you are sick, you will not be permitted and will have to send a new deposit and re-book.

Agreement

  1. I have self-assessed for COVID and any other contagious illnesses/sickness and declare myself in good health.

  2. To my knowledge, I do not have any mental or medical impairment or disability that might affect my well being as a direct or indirect result of my decision to have any tattoo procedure done at this time.

  3. I understand that the aftercare of my new tattoo is my responsibility. Any improper healing of the tattoo due to my not following the provided instructions is my own responsibility, and not that of Acoustic Ink or the tattooist.  Any touch-ups needed due to aftercare neglect are at the going studio rates.

  4. I understand and agree to not touch the tattoo during the tattooing session.

  5. I understand that if I do not stay still during the tattoo procedure, it may affect the results of my tattoo (wiggly lines, stray dots, depth, etc). If I continue to be unable to stay still, I understand that my tattoo artist may terminate the tattoo session.

  6. The one included touch-up is not applicable if I have neglected to follow instructions and care for my tattoo properly. The one touch-up must be completed within 6 months of the initial tattoo session.

  7. I understand that each person and body area is unique and may react differently than another. Skin tone, texture, condition, and location all respond to being tattooed differently. I agree that my artist has discussed this with me in regards to my tattoo(s).

  8. I understand that the finished tattoo may vary somewhat in appearance, colour and/or detail from the drawn design. I will take adequate opportunity to ensure I am happy with the placement and design of the tattoo.

  9. I acknowledge it is not reasonably possible for the representatives and artists of Acoustic Ink to determine whether I might have an unknown or undisclosed allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible.

  10. Being of sound mind and body, I hereby release any and all employees, agents, or persons representing Acoustic Ink from all responsibility. I agree not to sue Acoustic Ink or its heirs or assigns in connection with any and all damages, claims, demands, rights and causes of action of whatever kind or nature based upon injuries or property damages to or death of myself or any other persons arising from my decisions to have any tattoo work at this time, whether or not caused by any negligence of Acoustic Ink employees. 

  11. I agree for myself, my heirs, assigns, and legal representatives to hold harmless from all damages, actions, causes of action, claim judgments, costs of litigation, attorney’s fees, and all other costs and expenses which might arise from my decision to have any tattoo work done by Acoustic Ink and any of the artists operating there.

  12. I am aware that the tattoo will be permanent and that it can only be removed with a surgical procedure, and that any effective removal may leave permanent scarring and disfigurement. This cautionary notice is required to be provided to me by the health department and I hereby acknowledge receipt of this formal notice.    

  13. I agree to pay for any and all damages, injuries, and legal fees to any persons and property belonging to Acoustic Ink or any other person to whom they may become liable contractually or by operation of law, caused by or resulting from my decision to have any tattoo work at Acoustic Ink.    

  14. I hereby grant irrevocable consent and authorize the use of any reproduction by Acoustic Ink, any and all photographs which are taken this day of me, negative or positive, without any compensation to me. All images, video, or live Internet stream shall become and remain the property of Acoustic Ink, solely and completely.

  15. I acknowledge I am over the age of eighteen and I have truthfully represented that the obtaining of a tattoo is my choice alone. I consent to the design and application of the tattoo and to any actions or conduct of the representatives and employees of Acoustic Ink necessary to perform the tattoo procedure, including ceasing work.  

  16. I swear or affirm and agree that the above information is true and correct. I have been provided with information and instructions on after care. I have been made aware that if I have any signs or symptoms of infection, such as swelling, pain, redness, heat, fever, unusual discharge, or odour to contact my physician.  

  17. I understand that if I have any skin treatments, laser hair removal, plastic surgery, other skin altering procedures, considerable changes in weight, or pregnancy it may result in adverse changes to my tattoo.

  18. I agree to remove all sharp objects from my person during my tattoo session and agree to cover the cost of replacing the tattoo table or chair due to damages caused by my failure to do so.

  19. I understand that Acoustic Ink does not offer or issue any refunds for services rendered, including the deposit.

  20. I have been given ample opportunity to ask any questions about Acoustic Ink, its tattooist, the studio, and the tattooing process. I am satisfied with the answers provided.  

  21. I acknowledge that myself or the tattooist may choose to stop the tattooing session for any reason, at any time.

  22. I understand that Acoustic Ink does not use, provide, or recommend the use of numbing creams. If I have applied numbing cream myself, I take complete and full responsibility to any and all negative reactions or results due to its use. I also understand that Acoustic Ink or its artists have the right to refuse to work on skin that has been numbed.

Rules, Regulations, Etiquette

  1. Acoustic Ink is a certified and licensed tattoo studio. It is maintained at or above the standards of, and inspected by, Interior Health Services of British Columbia.While at a convention or guest spot, those locations are responsible for maintaining their good standing with their respective health and safety boards. I will continue to maintain safe practices.

  2. This online waiver form for Acoustic Ink MUST be filled out prior to getting tattooed.

  3. No children (under 18) or pets allowed in studio.

  4. Wear comfortable, loose-fitting clothing with no sharp objects. Be mindful of the placement of your tattoo when selecting your clothing. White is not recommended.

  5. The artist will try to make the client as comfortable as possible during the tattooing session. However, the body positioning may be, at times, uncomfortable in order to achieve the best tattooing results. Exposure of sensitive areas may also be necessary, depending on the tattoo’s location. Best efforts to cover you to a comfortable level will be made. Please dress with the placement of your tattoo in mind.

  6. Sharp objects are to be removed in order to protect the table and equipment. If you damage the table, you may be required to replace it. A ripped table is unusable.

  7. Do not show up under the influence of drugs or alcohol. If you do, you will be made to leave.

  8. Be respectful of others in the studio and spaces. Don’t yell, scream, or harass anyone on the premises. If you do, you will be made to leave.

  9. It is the right, and discretion of the artist to refuse or to stop working on someone whenever they deem necessary.

  10. When there is a second artist in the Revelstoke studio, no guests are permitted due to lack of space. Otherwise, you may bring up to 2 friends with you, who must not disturb anyone in the studio, including yourself. The artist(s) retain the right to have anyone removed from the studio, including yourself.Bringing company to conventions or guests spots is to be discussed on a per-location detail basis.

  11. No open food or drink in the tattooing space (e.g., take-out meals, open-top beverages). Snacks and sealed beverages are ok. It's best to come hydrated and comfortably fed. Do bring a water bottle.

  12. Ensure you have a safe way to get home after your appointment in the case that your driving abilities are compromised.

  13. Tipping or gifting is always appreciated, but never expected. Referrals and Google Reviews are awesome!

  14. The following body areas are more prone to blowouts or need for touch-ups: wrists, hands, fingers, feet, ears, & behind ears, creases such as inside elbow and behind the knee.

  15. Your non-refundable deposit is attached to each design and its appointment(s). It is applied towards your tattoo fee when we complete your tattoo. It is not refunded or transferred if you cancel, don’t show up, or reschedule less than 24 hours in advance. Nor is it transferred if you change the design request completely and a new design must be started.

  16. Cash, e-transfer, and PayPal are accepted for payment. Payment in full is due before leaving the studio.

  17. You will be sent my aftercare sheet before your appointment. It is also available to download. It is your responsibility to care for your tattoo properly.

  18. One free touch up, within 6 months of the initial tattoo, is included. This does not cover client neglect in aftercare or changes of mind, it ONLY covers the quality of my work.

  19. Kata will not tattoo the inside of mouths, palms of hands, soles of feet, genitalia, or inner buttocks (crack), so don’t ask.

  20. The client may wear earbuds or engage in using their phone. It is preferred that phones be silenced and put away during the session. If the use of a personal entertainment device interferes with the tattooing process, it will be required to be put aside.

  21. SHOW UP ON TIME. For Revelstoke you have ONE 25-30 minute grace period to show up, after which you will be charged the going studio rate for my time. All other appointments have a 10 minute grace period, after which the going rate will be charged for my time.

  22. For ALL photos, you are required to ask me first, and to tag @acousticink.ca in any posts of my work.
